The case of a 20-year-old female patient, who presented with grade II obesity and metabolic complications, is reported in association with Wernicke's encephalopathy development following sleeve gastrectomy. A patient experiencing confusion, gait ataxia, and horizontal nystagmus two months post-surgery was seen at the Emergency Department. Her discharge included oral thiamine supplementation, along with a multidisciplinary rehabilitation program, as impairment in anterograde, retrograde, and working memory persisted. Two years subsequent to initial assessment, her dietary habits included a balanced, fractionated diet and vitamin supplements, demonstrating consistent compliance. An autosomal recessive genetic condition, Gaucher disease (GD), manifests as a lysosomal storage disorder. A 32-year-old woman, possessing no history of chronic conditions, was admitted to the facility for the treatment of weakness, bone pain, and abdominal pain. Her evaluation reported the presence of the following: hepatosplenomegaly, thrombocytopenia, osteoporosis, and anemia. The suspicion of Gaucher disease clinically was corroborated by glucocerebrosidase enzyme levels and genetic testing results. Her sister's comprehensive family screening further highlighted the presence of hepato-splenomegaly, alongside osteoporosis, thrombocytopenia, and anemia. No neurological symptoms were present in either sister. Sequencing the GBA1 gene in two patients produced a homozygous c.593C>A missense variant finding. In any previously published case, this variant has not been observed.
