A model of the human respiratory epithelium, robustly constructed in vitro, encompassing the alveolar and airway epithelia, is paramount for understanding the biological and pathological mechanisms within the human respiratory system. In a prior description, we detailed a process for deriving human lung organoids from the primary lung tissue. To generate mature alveolar or airway organoids, we now describe a bidirectional differentiation protocol. Organoid cultures of the lungs are expanded reliably for over a year, maintaining high stability. The resultant differentiated alveolar and airway organoids strikingly resemble human alveolar and airway epithelium, both morphologically and functionally, almost reaching physiological levels. We thereby devise a powerful and enduring organoid culture system that covers the complete human respiratory epithelium. It is the first two-phase bipotential organoid culture system, allowing for prolonged expansion and both-ways differentiation of respiratory epithelial cells. Expandable lung organoids, both long-term and differentiated, create a consistent and replenishing supply of respiratory epithelial cells, allowing for the replication and expansion of the human respiratory epithelium in culture. By providing a unique, physiologically active in vitro model of the human respiratory epithelium, the respiratory organoid system allows for a range of applications including respiratory viral infection research, disease modeling, drug testing, and preclinical trials. Eczema patients' and caregivers' choices concerning clinical trial participation (CTP) remain a poorly understood area of study, lacking clear knowledge of influential factors. This study examines the factors considered important for CTP by adult patients and caregivers, further analyzing the existence of differences between their perspectives.
The eczema survey, comprising 46 questions, was distributed to adults and caregivers of children during the period May 1st through June 6th, 2020. A study investigated the perceived importance of several factors in the context of CTP; this included a comparative study of responses from adults and their caregivers.
Eleven of the thirty-one assessed factors revealed a notable divergence in importance ratings between the adult cohort (n=470) and the caregiver cohort (n=134). A data set was compiled from 15 community residents who had experienced a stroke and had upper extremity hemiparesis. In this study, a personalized UE self-training program, lasting four weeks, leveraged motivational interviewing (MI) and ecological momentary assessments (EMA) for heightened engagement. The research comprised three stages: 1) interventionist MI training, 2) customized treatment plan development through shared decision-making, and 3) a four-week UE self-training program.
We will synthesize recruitment and retention rates, the implementation of the intervention, acceptance, adherence levels, and safety data in order to evaluate its feasibility. The intervention's effects on upper extremity (UE) status will be objectively measured, using quantitative approaches like the Fugl-Meyer Assessment, Motor Activity Log, Canadian Occupational Performance Measure, and bilateral magnitude ratio. Qualitative data, gathered from 11 semi-structured interviews, will provide insight into participants' perceptions and experiences related to the intervention. To develop a more comprehensive understanding of the factors that promote and obstruct engagement and adherence to UE self-training, quantitative and qualitative data will be interwoven.
The implications of this research will extend to improving scientific understanding of how motivational interviewing and electronic monitoring of adherence can improve engagement and adherence in upper extremity self-training for stroke rehabilitation. Through this research, a measurable improvement in UE recovery will be observed in stroke survivors readapting to community life.

Leveraging background information, peer teaching is a powerful educational approach frequently adopted in medical school curricula. First-year medical students, in previous iterations of the program, used the gross anatomical structures they had dissected in the anatomy lab to teach their peers. Though this strategy provided a platform for students to learn from each other, it unfortunately resulted in the unintended consequence of not engaging every student. Acknowledging these observations, and the need to limit student enrollment in the lab due to the COVID-19 pandemic, a method was put in place to allow students to engage in virtual anatomy peer instruction. The objective was to create a virtual platform enabling students to effectively and efficiently learn and teach each other. Students, organized into teams of four, were given the assignment of identifying and labeling 4-5 designated anatomical structures on cadaver-based images, along with a justification for their selections, a pertinent discussion point related to each structure, a 5-minute video presentation encapsulating these steps, and finally, a critical review and constructive feedback session on another team's presentation.
